Gout - Treatment and Prevention

The Treatment and Prevention Guidelines of Gout is very important to know and to understand when dealing with the effects of Gout. Good Information is key to knowing what to do when the onsets of foot issues occur, especially Gout. Below, we will go through both the Treatments and The Prevention options for Gout.





Treatments - Most of the treatments for Gout involve medications that can help to alleviate the pain associated with Gout. Most of the medications given to the patient by a doctor will take down and get rid of most of the Uric Acid in the body which causes Gout. Here are some of the other treatment options:

  • Nonsteroidal Anti-inflammatory Drugs (NSAIDs) - Drug given by your doctor to control the inflammation and pain of Gout. NSAIDs also include over the counter medications such as Advil, Motrin, and Aleve. 

  • Colchicine - If you cannot take NSAIDs, then you may be prescibed Colchicine by your doctor which effectively reduces Gout Pain, especially right after the symptoms of Gout occur. 

  • Corticosteroids - Drugs such as Prednisone can be given to patients of Gout, and can also take down inflammation of Gout.

  • Medications that Take Down Uric Acid Level




Prevention - 

  • Keep Fluid Intake High - Drink lots of water, and avoid sugary drinks.

  • Limit or Avoid Alcohol

  • Eat a Balanced Diet - Lots of Fruits, Vegetables, Whole Grains, Low-Fat Milk

  • Limit Intake of Meat, Fish, and Poultry

  • Maintain a Healthy Weight

Gout - Causes and Symptoms

As talked about yesterday, Gout is a complex form of arthritis which affects the joint at the base of the big toe.



Causes of Gout - According to the Mayo Clinic, Gout occurs when urate crystals accumulate in your joint, causing the inflammation and intense pain of a gout attack. Urate crystals can start to form when a person has a high amount of uric acid in their blood. Uric Acid is produced in your body naturally when it breaks down purines which are found in certain foods, such as anchovies, asparagus, and mushrooms. Usually, uric acid dissolves in your blood and passes through your kidneys into your urine. However, sometimes your body produces too much uric acid or your kidneys excrete too little uric acid. When this happens, uric acid can build up, forming sharp urate crystals in a joint or surrounding tissue that cause pain, inflammation and swelling. About 90% percent of the time Gout is found in the base of the big toe.




Symptoms of Gout - The symptoms of Gout are most often acute, occurring at night, and sometimes without any warning. The most common symptoms of Gout are as follows:

  • Intense Joint Pain - Gout usually occurs in the big toe, but it can occur in your feet, ankles, knees and hands as well. The pain is most likely to be worst within the first 12 to 24 hours after it begins.

  • Lingering Discomfort - After the most severe pain from Gout occurs (first 12-24 hours), some of the joint discomfort may last from a few days to even a couple weeks.

  • Inflammation and Redness - The affected joint or joints become swollen, tender, and red.

Information and Facts on "Gout"

Gout is a very painful complex form of arthritis in the foot that can affect anyone who has the predisposition for the issue. According to the Mayo Clinic, Gout is characterized by sudden, severe attacks of pain, redness, and tenderness in joints, often the joint at the base of the big toe. The Metatarsal-Phalangeal Joint at the base of the big toe is the most commonly affected area (50%) of the time. An Acute gout attack can make it seem as if your big toe is on fire. The joint is so tender, that even the weight of a bed sheet touching the affected person's toe can be intolerably painful. The good news is that Gout is treatable and there are different ways to treat the symptoms of gout from flaring up to much.

Many people suffer terrible foot pain/ankle issues but do not know where to turn. There are "many" over the counter remedies such as Dr. Scholl's Gel Insoles to try to counteract the pains of mild to serious medical conditions that go un-diagnosed. If you suffer pains in the foot area and it continues to linger it might be wise to finally bite the bullet and go see a podiatrist. According to the APMA (American Podiatric Medical Association) a podiatrist is a doctor of podiatric medicine (DPM), also known as a podiatric physician or surgeon. Podiatrists diagnose and treat conditions of the foot, ankle, and related structures of the leg.




What Are The Qualifications Of a Podiatrist? -
According to the APMA website, podiatrists are the most qualified doctors to care for your feet. They complete four years of training in a podiatric medical school and three years of hospital residency training. This training is similar to that of other doctors. Podiatrists can specialize in many fields, including surgery, sports medicine, wound care, pediatrics (children), and diabetic care.

Are Podiatrists Board Certified?
Podiatrists can earn board certification with advanced training, clinical experience, and by ultimately taking an exam. The American Board of Podiatric Medicine and the American Board of Podiatric Surgery are the certifying boards for the field.

Soft Sole - (Diabetic/Arthritic)
Constructed using only premium quality materials to create an exceptionally low bulk soft device for those diabetic or arthritic patients needing bio mechanical support. Provides shock absorption and protection of tissues against pressure and impact.  Materials Include: Low bulk polypropylene Shell to fit in a wider variety of shoes. A Full length pink plastazote comes standard with this appliance. Rear foot posting is intrinsic. NOTE: Perf Rx or Spenco may be substituted for the full length Plastazote.


Flexor 5000 - Constructed using only premium quality materials to create an exceptionally soft device for those diabetic or arthritic patients needing bio-mechanical support. Provides increased shock absorption and protection of tissues against pressure and impact. Materials Include: Thin Polypropylene Shell (less than 1.2mm) sandwiched between a full length 1/8" Perf Rx top cover and a full length padded bottom cover. Sports Orthotics

NewGen Advanced Orthotics Lab offers a wide variety of different types of orthotics for different needs. For Athletes or Runners New Gen Orthotics Lab makes three different types of orthotics. Below are the three different types of sport orthotics we offer, and description of each ( All Star, Slim Sport, and Court Time).




All Star - (Long Distance Running-cycling-walking)
Materials: Poly Pro shell made to exacting standards of rigidity and bulk. A sulcus or full length padded Swirl top cover comes standard with this appliance as well as extrinsic crepe padded rear foot posts. Note: A perf Rx or similar padded top cover may be substituted for Swirl at no additional charge.


Court Time - (All Court and Racquet Sports)
Materials: Designed to be used by professional basketball teams, this sophisticated appliance is crafted with a full length 1/8" Perf Rx top cover over a 2.9mm Poly Pro Shell. A full length 1/16" Cloud padded forefoot extension and a dual density crepe EVA mid arch fill are included.


Slim Sport - (Cleats and Low Heel Seats)
Materials: Intrinsic rear foot design includes a low bulk Poly Pro Shell for those "problem" shoes or cleats where an extrinsic rear foot posted orthotic just won't fit. Standard materials include a full length Swirl top cover and a full length cloud bottom cover or full extension with your choice of padding.

"Choosing" The Correct Orthotic For Runners

As stated in the following article a good orthotic for running is similar to a Jaguar vehicle. If you find the correct model it's heaven, but if you end up with a Pinto, the foot problems not only remain, but could actually get progressively worse. NewGen Orthotics Lab strives to make sure that the patient will always be comfortable in their orthotics, but that they will always be getting better too. A Look At Plantar Fasciitis

Many people suffer from terrible foot pains in just their average everyday life. If the foot pains they experience continue to linger or get worse it might be wise to go see a specialist in this field which would be a podiatrist. Of all the different types of ailments a patient suffers from, one if not the most common chronic foot problem a patient deals with is plantar fasciitis. Plantar Fasciitis causes sharp pain on the plantar surface of the foot due to the inflammation of the plantar surface area shown below.

According to the National Library of Medicine, Plantar Fasciitis is inflammation of the thick tissue on the bottom of the foot. This tissue is called the plantar fascia. It connects the heel bone to the toes and creates the arch of the foot.

Out of all of these plans for treatment of Plantar Fasciitis the most common treatments for success are the use of custom orthotics (from a podiatrist), and also making sure to continue to wear good support and cushioned shoes. A good custom made orthotic, in the majority of the cases should provide good support, by influencing the foot position to a more comfortable and natural state. Non-Surgical Treatment for Bunions

Yesterday, we spoke of the surgical procedures than can be accomplished through surgical means to make sure to get rid of pesky , and sometimes very painful bunions on the foot. Although surgically getting rid of bunions usually helps a lot, its usually the last resort because it does require surgery. Most people only get a bunionectomy (surgery for bunions) , when the non-surgical treatments, and/or remedies do not work. Below are some examples of the non-surgical techniques that a patient should ALWAYS try before considering surgery.


  1. Roomier Shoes - Starting fresh with a new pair of "Roomy" shoes in many cases can cause the bunion to lesson. Many times bunions begin to form due to no room left in the toe area and thus the toes crunching up together and causing joint problems such as bunions. Be advised please to get a shoe that has room in the toes and that lets your feet and toes have some "breathing room" to help with bunions.
  2. Padding and Taping - Adding extra padding and taping (most cases put on by podiatrist) can alleviate and in many cases help align your toes into better positioning so your bunion does not worsen, and the pain is alleviated. It is best to find out the proper tape and padding to put on by either going to see a trained podiatrist, or by also doing your own good research.

  3. Medications - Medications such as Ibuprofin , Tylenol, Aleve, and Advil can help to take away some         of the pain temporarily, and in many cases is a great way to take down some of the pain associated with       bunions and the joints of the toes.

  4. Custom Orthotics - Orthotics are a good way of helping with the effects of a bunion by helping to               the foot in many cases to the proper positioning, including the toes. Making sure to get a good custom         orthotic is wise however, because a custom orthotic is molded specifically for "your" foot.

Bunionectomy Surgery

Yesterday, I spoke to the issues and causes of bunions. Many of the treatments offered for bunions are able to be accomplished without surgery. Treatments such as extra padding, roomier shoes, and Orthotics, can quite often relieve and fix the problems associated with painful and often large bunions.


However, if these traditional non-surgical procedures used do not help to reduce or get rid of the the bunion, many times surgical procedures must be undergone to get rid of a bunion. Bunion Surgery, also called a "bunionectomy", involves an incision by a podiatrist in the top or side of the big toe joint, and removal and re-alignmment of soft tissue and bone according to WebMD website. This surgery is done to relieve pain and to restore alignment to the joint. If the joint is SEVERELY deformed though, wires, plates, or screws can be used to make sure proper alignment lasts. A bunionectomy usually lasts for an hour or more and is done in an outpatient facility for the most part.



As with any surgery, there is always some risk involved so a bunion surgery should usually be done only when the over the counter/non surgical remedies do not work. A good doctor should with lots of good reviews of corrective surgeries should always be checked out as well so you make sure the best doctor with a high level of surgical skill will be working with your problem area. Bunions

According to the Mayo Clinic a "bunion" is an abnormal, bony bump that forms on the joint at the base of your big toe. Bunions start to form when your big toe pushes up against your other toes, causing the joint in the big toe to move in the opposite direction it is supposed to move. Over time, a bunion can start to get larger, and start to be more physically pronounced, as a ball shaped bump on the other side of your big toe. A smaller bunion, also known as a "bunionette" can also form but not be as large as a large bunion. If you feel like you have these symptons, even if it seems it might be at the beginning stages, it might be wise to contact a foot podiatrist or someone in the foot professional field, to take a look at what is going on with your foot.

Causes of Bunions: 
  • Wearing Shoes Too Tightly (High Heels in many Cases)
  • Arthritis
  • Inherited Foot and Joint Problem
Symptons:
  • A large bump on the oustside of your big toe
  • Swelling, Redness around your big toe
  • Restricted movement of your big toe 



Treatments: 
  • Changing Shoes - Wearing more comfortable and Roomy Shoes
  • Padding and Taping - Can get help from a podiatrist for this
  • Medications: Tylenol, Advil, Aleve can in many cases alleviate the pain of bunions for some time
  • Orthotics - Many times a good orthotic can help move the feet and toes into correct position
  • Surgery - Bunionectomy is a surgical procedure done by a doctor that can go into the bone and joint area to fix the bunion problem area. As with other surgeries there always is a slight risk involved.
